The new Patch 1.1.0a arrives before the Season of the Malignant, which has a bunch of gameplay changes... one of the biggest is making the Altar of Lilith unlocks available account-wide. Blizzard has also rebalanced a bunch of the classes in Diablo IV, with the Sorceress getting nerfed with the Devouring Blaze skill which many gamers use. Not only that, but there are monster movement changes, modifications to XP progression, accessibility, and more.
Blizzard has a bunch of new Unique items and Legendary Aspects inside of Sanctuary, where you can get your hands on the new Unique items from World Tier IV, while the Legendary Aspects can be acquired through any World Tier. Blizzard has a huge article on the Diablo IV patch notes, which you can read here.
